Hello, and welcome to my Doctor of Nursing Practice (DNP) Project. I, Hana Kleinova CRNA, am the principal investigator, and my faculty sponsor, Dr. Rebecca Lee. I invite you to take a brief educational survey regarding Knowledge Gain in Optimized Protamine Regimen Training.

I understand what this study is about, that my participation is voluntary, and that I can end my participation at any time without penalty. Completion of the survey will indicate my consent to participate.

You are asked to complete an online video and questionnaire that takes approximately 15-20 minutes. No personally identifying information will be collected (e.g., IP address). This survey is designed only to collect anonymous data.

There are minimal physical, psychological, and or social risks to participants. The associated risks are no higher than if the participant were to watch a YouTube video. While there is no direct benefit to you, your information may contribute to quality improvement in anesthesia cardiac practice.

Data collected via Survey Monkey is de-identified and will be reported as an aggregate. All data will be recorded electronically on a password-protected computer that will only be accessible to the research team members, including the faculty sponsor. The project data will be kept for five years as per IRB guidelines. After five years, electronic data will be deleted from the computer and the recycling bin. All information will be protected as required by the Institutional Review Board, which oversees all University research. We may use or share your research information for future research studies. If we share your information with other researchers, we will not share any information that can directly identify you. These future studies may be similar to this study or completely different. We will not ask for your additional consent to use the de-identified data in future studies. Participants can elect not to participate in the email survey. In addition, participants may elect to remove their responses at any time without penalty. Selecting to participate or not participate will not result in any harm to the participant. There is no compensation for participating in this survey.
